This model withdrawal form is intended for consumers who wish to exercise a statutory right of withdrawal for an eligible distance contract. The right generally lasts 14 calendar days from conclusion of a service contract, subject to statutory exceptions.

Service ordered; order reference; order date; consumer name; consumer address; date; signature only if this form is sent on paper.

5. Early commencement of service

Where you expressly requested that the service start before the withdrawal period expired, a proportionate payment for services actually provided may be due if the legal conditions are met. Payment alone does not waive withdrawal rights.
